Dear brothers and sisters,

I do hereby certify that I am Mohamed Mustafa Mohamed Aflal, holder of NIC No. 197513B602048, residing at No. 33A, School Lane, Sarikkamulla, Panadura.

I am a father of 4 schooling children. I was doing a hardware business before but am currently unemployed since 3 years due to my illnesses. I am a severe patient of diabetic retinopathy (to the stage of losing my eyesight), a chronic heart disease, diabetes, and hypertension. Now I am suffering from the end stage of renal failure. Thus, my nephrologist has suggested a kidney transplant as early as possible. The average cost estimate for a surgery of this type is LKR 6.5 million, including all pre- and post-operative conditions and the donor. We are currently living in a rented house.

Herewith, I plead for an urgent request from your organisation to help me financially to complete my required necessity as early as possible.
